Exhibitor registration forms must be completed Registrations submitted after this date will be subject to a $100 late fee.
Cancellations received on or before September 10, 2012, will be subject to a $25 cancellation fee. No refunds will be made for cancellations received after September 10, 2012. Cancellations must be in writing and sent via fax or email.
*Ads must be submitted via email to patriciabanas@comcast.net by September 1, 2012 as a jpg or gif file.

Exhibitor Display Opportunities
Attended Exhibit Display: $450
Display Space includes one 6’ skirted table, table top signage and one complimentary conference registration.
Additional Exhibit Display: $350
Unattended Display - $150
Unattended Display Space includes one 6’ skirted table to display your company catalog or printed materials, without a representative. WSLHA representatives will be available to set up these materials. Extra products and materials will be returned at the exhibitor’s expense. This level does not include one complimentary conference registration.
Please Note: Unattended Exhibit Displays are provided at this reduced fee based on availability of space in the Exhibit Hall.
Door Prize or Raffle Donation: No Charge
Donate a door prize for drawings or for our annual raffle conducted in the Exhibit Area during the Friday Evening Reception. Please send door prize items to:
